REQUEST FOR QUOTATIONS(RFQ) FOR THE SUPPLY OF MEDICAL LABORATORY CONSUMABLES FOR NORTHEAST OPERATIONS
(RFQ Reference: ACE-1-RFQ-MLC-007-001-2026)

(a) BACKGROUND
Achieving Health Nigeria Initiative (AHNi) is one of Nigeria’s leading national non-governmental organizations committed to promoting a healthy and safe society where everyone can thrive. Our mission is to improve the quality of life for all people, especially vulnerable groups, through inclusive, community-led programming.

AHNi hereby invites interested and qualified suppliers to submit sealed quotations for the supply of medical laboratory consumables for its Northeast operations.

(c) MINIMUM ELIGIBILITY REQUIREMENT:
1. Company Profile: Bidders must submit a comprehensive company profile indicating registered address(es), functional email address(es), telephone number(s), and details of the designated contact person (name and position).
2. Evidence of Legal Registration: Certificate of Incorporation issued by the Corporate Affairs Commission (CAC) for limited liability companies, or Certificate of Registration of Business Name for sole proprietorships, as applicable.
3. Latest Tax Clearance Certificate (2024 or later). Please note that AHNi will verify the authenticity of the TCC by scanning the QR code.
4. Evidence of Financial Capability: A signed copy of the most recent audited financial statements (2024 or later).
5. Bank Reference Letter duly addressed to the AHNi Procurement Committee.


6. Verifiable evidence of at least three years’ experience executing similar contracts, supported by copies of job orders/contracts and corresponding waybills, delivery notes, or job completion reports demonstrating successful execution.
7. Provision of a catalogue containing colour photographs of all Medical Laboratory Consumables proposed for supply. A single catalogue covering all applicable items listed across Lots 1–4 is sufficient.
8. Financial Quotation: A comprehensive financial quotation clearly stating the applicable commercial terms, including price, quotation validity period, delivery lead time, delivery cost, and warranty, where applicable.

(d) EVALUATION AND SELECTION CRITERIA:
Bidders must ensure that all requested information under section (c) is fully included in their submission documentation. Proposals will be evaluated based on the following criteria:

2ND STAGE – Conformity with all minimum eligibility requirements listed as eligibility criteria 1–6 above. | PASS OR FAIL
3RD STAGE – Evaluation of electronic samples submitted in accordance with Eligibility Criterion 7 | PASS OR FAIL
4TH STAGE -Financial Evaluation and Consideration for Award: Only bidders that meet all minimum eligibility criteria (1–7) will proceed to the financial evaluation stage. Financial proposals will be evaluated using the Quality and Cost-Based Selection (QCBS) method. | Award Consideration: The most competitive and responsive bid(s) will be considered for award. Evaluation and award decisions will be made separately for each Lot.

(e) SUBMISSION:
Submit one (1) original hard copy. Minimum Eligibility Requirements (1–6), the Electronic Sample Catalogue (Criterion 7), and Financial Proposals for Lots 1–4 must be submitted as separate documents enclosed within one sealed envelope clearly marked with Tender Reference No. “ACE-1-RFQ-MLC-007-001-2026.”
