INVITATION TO TENDER
1.0 INTRODUCTION
The Futa Teaching Hospital, Akure is established to provide qualitative, affordable, accessible healthcare services to our patients at the primary, secondary and tertiary levels with the ultimate goal of Excellence and Compassionate Care among Nigerians. Take off grants was appropriated for the Procurement, Equipping, Renovation and Construction of the infrastructure within Futa Teaching Hospital, Akure. In compliance with Public Procurement Act 2007, Reputable and Competent Companies with relevant experiences and good track records are hereby invited to indicate interest for the following:

3.0 ELIGIBILITY REQUIREMENTS
i. Evidence of Certificate of Incorporation with the Corporate Affairs Commission (CAC) including form CAC 1, CAC 2 and CAC 7.
ii. Evidence of company Income’s Tax Clearance Certificate for the last three (03) years (2023, 2024, 2025) valid till 31st December, 2026.
iii. Evidence of Pension Clearance Certificate valid till 31st December, 2026 (this requirement is only applicable to bidders whose number of staff is 15 and above).
iv. Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2026 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s annual turnover is N50m and above.
v. Evidence of Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2026.
vi. Evidence of registration on the National database of Federal contractors, consultants and service providers by inclusion of interim registration report (IRR) expiring on 31st December, 2026 or valid certificate issued by the Bureau of Public Procurement (BPP).
vii. Sworn Affidavit: Disclosing whether or not any officer of the relevant committees of the Futa Teaching Hospital, Akure, Ondo State or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particular.
viii. The company is not in receivership, insolvent or bankrupt;
ix. None of the directors has been convicted in any court of law for procurement fraud;
x. Reference letter from reputable commercial bank in Nigeria indicating willingness to provide credit facility for the execution of the project when needed;
xi. Company’s Audited Accounts for the last three (03) years 2023, 2024, 2025;
xii. Company’s profile with the Curriculum Vitae of Key Staff to be deployed for the project including copies of their Academic/Professional qualifications such as COREN, QSRBN, ARCON, CORBON, SURCON, NITDA and CPN, etc;
xiii. For Motor Vehicle’s procurement: only companies included in the National Automotive Design and Development Council’s approved list of local manufacturers of vehicles or their authorized representatives should bid for the vehicles;
xiv. Verifiable documentary evidence of at least three (03) similar jobs executed in the last five (05) years including letters of award, valuation certificate, job completion certificates and photographs of the projects.
xv. Health and Medical Equipment: Letter of Authorization from the Original Equipment Manufacturers (OEMs) listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria;
xvi. For Joint Venture/Partnership, Memorandum of Understanding (MoU) indicating Lead Partner should be provided (CAC, Tax Clearance Certificate, Pencom, Industrial Training Fund (ITF), NSITF and Interim Registration Report (IRR) and Sworn Affidavit are compulsory for each JV Partner).
xvii. Letter of consent to allow for verification of all the documents submitted including CAC, Tax Clearance Certificate, Pencom, Industrial Training Fund (ITF), NSITF and Interim Registration Report (IRR);
Note: All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.
4. COLLECTION OF TENDER DOCUMENTS
The interested companies are to collect the Standard Building Document (SBD) from the office of Head of Procurement Department on evidence of payment of a non-refundable Tender Fee of N10,000.00 per Lot, paid into the Futa Teaching Hospital Remita Account in any reputable Commercial Bank in Nigeria. The Hospital’s receipt and photocopy should be attached to the Tender Document.
