3 ELIGIBILITY REQUIREMENTS:
(a) Evidence of Certificate of Incorporation with the Corporate Affairs Commission (CAC) including form CAC1.1 or CAC2 and CAC 7 as well as current files of annual returns;
(b) Evidence of Company’s Income Tax Clearance Certificate for the last (3) years (2023, 2024, 2025) valid till 31st December, 2026;
(c) Evidence of Pension Clearance Certificate valid till 31st December, 2026 (this requirement is only applicable to bidders whose number of staff is 15 and above);
(d) Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2026 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s);
(e) Evidence of Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2026;
(f) Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2026 or valid Certificate issued by BPP;
(g) Sworn Affidavit disclosing whether or not any officer of the relevant committees of the Federal University of Health Sciences Ila-Orangun or the Bureau of Public Procurement is a former or present director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
(h) Company’s duly signed Audited Accounts for the last three (3) years 2023, 2024, 2025;
(i) Reference letter from a reputable Commercial Bank in Nigeria, indicating unconditional willingness to provide credit facility for the execution of the project when needed;
(j) Company’s profile and organizational structure with the Curriculum vitae of key directors and staff to be deployed for the Project with their roles, including copies of their Academic/Professional qualifications such as SURCON, COREN, QSRBN, ARCON, CORBON, etc;
(k) Verifiable documentary evidence of at least (3) similar jobs (in size, nature & complexity) executed in the last five (5) years (2022-2026) including serially arranged in-terms-of-year letters of awards, Valuation Certificates, Store Receipt Vouchers for goods, job completion certificate for works and Photographs of Projects;
(l) List of Equipment with proof of ownership or Lease Agreement for Works and Letter of Authorization from the Original Equipment Manufacturers for Goods as the case may be; and
(m) For Joint Venture/Partnership, Memorandum of Understanding (MoU) indicating Lead Partner should be provided (CAC, Tax Clearance Certificate, Pension Clearance Certificate, ITF Compliance Certificate, NSITF Clearance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner).

– that the company does not have existing loan or financial liability with any bank, other financial institution or third party that is classified as doubtful, non-performance, bad or whose repayment portion has been outstanding for the last four (4) months;
h. Company’s Audited Accounts for the last Three (3) Years 2023, 2024, 2025 duly acknowledged by the Federal Inland Revenue Service (FIRS);
i. Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
j. Company’s Profile with the Curriculum Vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional Qualifications (In case of construction: COREN, QSRBN, ARCON, CORBON etc);
k. Verifiable documentary evidence of at least Three (3) Similar Jobs Executed in the Last Five (5) Years Including Letters of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the Projects;
l. Works: List of Plants/ Equipment with Proof of Ownership/Lease Agreement;
m. Lot A3: Only companies included in the National Automotive Design and Development Council’s approved list of local manufacturers of vehicles or their authorised representatives should bid;
n. Lot A4: Evidence of current Licence/accreditation with Computer Professionals Registration Council of Nigeria and National Information Technology Development Agency (NITDA);
o. Lot A5&A6: Letter of Authorization from the Original Equipment Manufacturers listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria;
p. Lot A7: Evidence of Firm’s current Licence issued by Pharmacists Council of Nigeria or other related professional body(ies);
q. Lot A9: Only Company’s or Individual Consultant’s with relevant experience in the field with Curriculum Vitae of key staff to be deployed for the service and Evidence of Firm’s registration with relevant professional body(ies);

2.0 PREQUALIFICATION/ ELIGIBILITY REQUIREMENT
All interested companies are to submit copies of the following documents and the original to be made available for sighting during Due diligence exercise.
a. Evidence of Certificate of Incorporation issued by the Corporate Affairs Commission (CAC) including form CAC 1.1 or CAC 2 and CAC 7.
b. Evidence of company’s Income Tax Clearance Certificate (TCC) for three (3) years valid till 31st December, 2026;
c. Evidence of Pension Clearance Certificate as provided in Section 16 Subsection 6 (d) of the public procurement Act, 2007 valid till the 31st December, 2026. (This requirement is only applicable to bidders whose number of staff is 15 and above in line with the PENCOM ACT. However, bidders with less than 15 (fifteen) staff are to provide a Sworn Affidavit to that effect).
d. Evidence of Industrial Training Fund (ITF) Compliance Certificate in Compliance with ITF Act, 2011 as amended valid till 31st December, 2026 (This requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s annual turnover is 50M and above. However, bidders with less than 5 (five) staff or the bidder’s annual turnover less than 50M are to provide a Sworn Affidavit to that effect).

l. For Joint Venture/Partnership, Memorandum of Understanding(MoU) indicating Lead partner should be provided (CAC, Tax Clearance Certificate, Pension Clearance Certificate ITF Compliance Certificate, NSITF Clearance Certificate, IRR and Sworn Affidavit are compulsory for each JV partner);
m. Letter of authorization as representatives of the Original Equipment Manufacturers (OEMs);
