1.0 INTRODUCTION
The Futa Teaching Hospital, Akure is established to provide qualitative, affordable, accessible healthcare services to our patients at the primary, secondary and tertiary levels with the ultimate goal of Excellence and Compassionate Care among Nigerians. Take off grants was appropriated FOR THE PROCUREMENT, EQUIPPING, RENOVATION AND CONSTRUCTION OF THE INFRASTRUCTURE WITHIN FUTA TEACHING HOSPITAL, AKURE. In compliance with Public Procurement Act 2007, Reputable and Competent Companies with relevant experiences and good track records are hereby invited to indicate interest for the following:

3.0 ELIGIBILITY REQUIREMENTS

i. Evidence of Certificate of Incorporation with the Corporate Affairs Commission (CAC) including form CAC 1, CAC 2 and CAC 7.
ii. Evidence of company Income’s Tax Clearance Certificate for the last three (03) years (2023, 2024, 2025) valid till 31st December, 2026.
iii. Evidence of Pension Clearance Certificate valid till 31st December, 2026 (this requirement is only applicable to bidders whose number of staff is 15 and above).
iv. Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2026 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s annual turnover is N50m and above).
v. Evidence of Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2026.
vi. Evidence of registration on the National database of Federal contractors, consultants and service providers by inclusion of interim registration report (IRR) expiring on 31st December, 2026 or valid certificate issued by the Bureau of Public Procurement (BPP).
vii. Sworn Affidavit: Disclosing whether or not any officer of the relevant committees of the Futa Teaching Hospital, Akure, Ondo State or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particular.
viii. The company is not in receivership, insolvent or bankrupt;
ix. None of the directors has been convicted in any court of law for procurement fraud;
x. Reference letter from reputable commercial bank in Nigeria indicating willingness to provide credit facility for the execution of the project when needed;
xi. Company’s Audited Accounts for the last three (03) years 2023, 2024, 2025;
xii. Company’s profile with the Curriculum Vitae of Key Staff to be deployed for the project including copies of their Academic/Professional qualifications such as COREN, QSRBN, ARCON, CORBON, SURCON, NITDA and CPN, etc;
xiii. For Motor Vehicle’s procurement: only companies included in the National Automotive Design and Development Council’s approved list of local manufacturers of vehicles or their authorized representatives should bid for the vehicles;
xiv. Verifiable documentary evidence of at least three (03) similar jobs executed in the last five (05) years including letters of award, valuation certificate, job completion certificates and photographs of the projects.
xv. Health and Medical Equipment: Letter of Authorization from the Original Equipment Manufacturers (OEMs) listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria;
xvi. For Joint Venture/Partnership, Memorandum of Understanding (MoU) indicating Lead Partner should be provided (CAC, Tax Clearance Certificate, Pencom, Industrial Training Fund (ITF), NSITF and Interim Registration Report (IRR) and Sworn Affidavit are compulsory for each JV Partner).
xvii. Letter of consent to allow for verification of all the documents submitted including CAC, Tax Clearance Certificate, Pencom, Industrial Training Fund (ITF), NSITF and Interim Registration Report (IRR);

Note: All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.

4. COLLECTION OF TENDER DOCUMENTS
The interested companies are to collect the Standard Building Document (SBD) from the office of Head of Procurement Department on evidence of payment of a non-refundable Tender Fee of N10,000.00 per Lot, paid into the Futa Teaching Hospital Remita Account in any reputable Commercial Bank in Nigeria. The Hospital’s receipt and photocopy should be attached to the Tender Document.

5. SUBMISSION OF TENDER DOCUMENTS
Prospective bidders are to submit bid for each of the Lot desired, two (02) hard copies each of the technical and financial bids with softcopy of financial bid only in MS Excel format, packaged separately in sealed envelope and clearly marked as “Technical Bid” or “Financial Bid”. Thereafter, put the two sealed envelopes together in a bigger sealed envelope addressed to The Chief Medical Director, Futa Teaching Hospital, Akure, Ondo State and clearly marked with the name of the project and the Lot number. Furthermore, the reverse of each sealed envelope should bear the name and address of the bidder, the completed and sealed tender documents should be dropped in the Tender Box in the office of the Chief Medical Director, Futa Teaching Hospital, Akure not later than 12:00 noon on Monday 5th October, 2026.

6. OPENING OF TECHNICAL BIDS
Only the technical bids will be opened immediately after the deadline for submission at 12:00 noon on Monday 5th October, 2026 at the Board Room, Administrative building, of Futa Teaching Hospital, Akure, Ondo State, in the presence of bidders or their representatives, while the financial bids will be kept un-opened. Please, ensure that you sign the Bid Submission Register in the Chief Medical Director’s Office at the Futa Teaching Hospital, Akure, Ondo State will not be held liable for misplaced or wrongly submitted bids. 7. GENERAL INFORMATION:
a. Bids must be in English Language and signed by an official authorized by the bidder;
b. Bids submitted after the deadline for submission would be returned un-opened,
c. All cost will be borne by the bidders,
d. Bid Documents can be collected in soft copies and Bidders can submit through Post (Courier Service) but the Hospital would not be liable to delivery at wrong address;
e. Note that Due Diligence will be carried out on all the claims made;
f. The technical bids will be opened in the presence of Bidders or their representative, CSO’s in the area of Anti-Corruption and Transparency and Private Professional bodies;
g. Only pre-qualified bidders at technical evaluation will be invited at a later date for financial bids opening, while the financial bids of un-successful bidders will be returned un-opened;
h. The Futa Teaching Hospital, Akure, Ondo State is not bound to pre-qualify any bidder and reserves the right to annul the Procurement process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act, 2007.
i. All pages of the bidding document must be signed.
